1. Please notify the health office of students: injuries, stitches, casts, crutches, sprains & illnesses as well as concerns/and or conditions regarding your child. 2. A doctor’s note is required to excuse a student from gym and/or outdoor recess post injury/illness. In addition a doctor’s release is required to resume participation after a medical excuse. 3. Please submit a doctor’s note to have on file in the health folder for all extended illnesses.

ILLNESS: 1. Students will be sent home with a fever (100 F or greater) and should be fever free for 24 hours without fever reducing agent such as Motrin, Advil etc. before returning to school. 2. Students with vomiting/diarrhea will be sent home. May return to school when they are symptom free and are able to retain liquids and solid food. 3. Children with an undiagnosed rash will be sent home from school. A note from a physician will be needed prior to the student returning to school. |

MEDICATION: 1 .The Medication Administration Form must be completed and signed by both the doctor and parent and on file in the health office before a student may receive, possess, or consume prescription or non prescription medication at school. Non prescription medication includes all over the counter medication as well as cough drops. 2. Dispensing of medication to the student by his/her parent must occur in the health office. 3. Please contact the health office for more information regarding medication at school. |

HEALTH EXAMINATIONS: State law requires physical health examinations & required immunizations for all students entering kindergarten and sixth grade. Health examination & immunization forms must be submitted to the health office by October 15th of the current school year or the student shall be excluded from school. All new students transferring into the district, completed physical examinations & required immunizations must be at school by October 15th or 30 days after their start date of student will be excluded. |

DENTAL EXAMINATION: February – Dental Month All Illinois children in kindergarten, second, sixth grades and for all out-of-state or out-of-country transfer students are required to show proof of an examination by a licensed dentist prior to May 15th of the school year. Dental examination completed within the previous 18 months of the May 15th deadline is acceptable. Proof of exam is required by May 15th of the current school year. Documentation that a dental examination will take place within 60 days after May 15 is acceptable. Dental examination waiver or an exemption based on religious grounds that meets IL State Board if Education Administrative Rules is acceptable. |

VISION/HEARING SCREENING Each school year, District 15 nurses conduct vision & hearing screening on students. The screening is generally conducted in the fall. You may request a screening at any time during the school year by calling the health office. Annual screening is provided to students in the following grades/groups:

EYE E XAMINATION: Children enrolling in kindergarten and enrolling in Illinois schools for the first time need to provide report of an eye examination to the health office by October 15 or 30 days after the students start date. Student wearing glasses are required to have a vision report form on file every two years. Optimally, students wearing glasses should have yearly eye exams. |

May – Vision Month School screening can detect visual problems, however a comprehensive eye exam by an eye doctor or optician may detect vision problems missed in routine screening. Common signs & symptoms of vision problems are 1) squinting or closing one eye while doing certain tasks 2) holding objects too close or sitting close to the television 3) headache complaints, especially at the end of the school day or after reading for a while 4) reversing words, letters or numbers when reading or writing.
